    PRP stands for Platelet Rich Plasma, means blood plasma that is rich in platelets (thrombocytes). These platelets are concentrated in the blood plasma when a blood sample is centrifuged.

    They are then injected for treatment of various painful conditions in tendons and joints. The method has attracted considerable interest over the last 10-15 years and its use is gradually increasing.

    The effect of blood platelets in this context is not fully understood, but it is known that they, among other things:

    • stimulates stem cells (MSCs) to mature into cartilage and bone cells
    • reduce inflammatory markers and joint swelling
    • reduces abnormal ‘enlargement’ of cartilage cells, which is a known stage in the arthritic process

    Scientifically, the treatment has been studied to varying degrees in different pain conditions. The strongest evidence appears to be in the treatment of osteoarthritis of the knee, where a very large number of studies show better and longer-lasting effects than saline (placebo), hyaluronic acid or cortisone injections.

    Indications

    Radiologically verified mild to moderate knee osteoarthritis (Kellgren-Lawrence 1-3).

    Persistence of significant symptoms (pain/soreness/swelling/impairment of function) despite adequate standard osteoarthritis treatment.

    Surgery, angular surgery or knee replacement, not considered.

    Contraindication

    Active or past cancer.

    Relative contraindications

    Treatment with Waran or other potent anticoagulants.

    Active intake of anti-inflammatory tablets, NSAIDs (thus one should stop this a few days before treatment and keep up throughout treatment).

    Side effects/risks

    No serious side effects have been reported. The risk of infection with joint injections is very low. The side effects that may occur according to the literature are generally non-specific and transient, such as pain, stiffness, swelling, dizziness, nausea, headache, sweating, palpitations. In our experience, these side effects are very rare.

    During treatment

    If everything feels fine, you can live as usual. However, it is recommended to avoid heavy/strenuous exercise for the legs during the treatment period.

    Results

    The majority of studies show good results, but a few studies do not show a definite effect. The main difficulty in assessing results and comparing studies is the wide variation between different types of PRP preparation and treatment regimens. Patient composition and degree of osteoarthritis also differ between studies. Most studies suggest that it takes a few weeks to achieve treatment effects, which include reduced pain/pain and swelling and improved function. The effect can last for 6-24 months according to different studies. There is some scientific and empirical support that a repeat dose after 6 or 12 months may prolong the effect in case symptoms become more pronounced again at this time.
